Patients Rights
- To be treated in a courteous, civil and co-operative manner.
- A clear explanation of, and advice on, their medical condition and treatment.
- A referral for specialist medical opinion or other second opinion when appropriate
- Access to their own medical records, in accordance with the Data Protection Act – please ask the Practice Manager for details
- Request to be seen by a specific doctor or health care professional where possible.
- Patient’s aged 16-74 may have a health check every 3 years by request
- Patients over 75 may have an annual health check, at home if necessary.
“What the Practice asks from our Patients”
- Do not ask for information about anyone other than yourself
- Tell us of any change of name, address or telephone number, so that our records are accurate
- Please cancel your appointment if you are unable to attend
- Please be punctual, but be prepared to wait if your own consultation is delayed by an unexpected emergency
- Please allow sufficient time for your consultant’s letter or the results of any tests to reach us
